Census Information
In order for us to provide you a group health insurance quote, we will need to collect some information about your company. Below we have two different ways to provide this information. We have a PDF census that you will print out and fill in or an excel that can be done on your computer with the file emailed to us. The PDF census version is here. This will allow you to print out the form, hand write the info and fax over to us.
